Common Situations When Your Information is Shared


  • With your insurance company or Employee Assistance Program (not your employer) to bill for payment of services or the insurance company audits our files to make sure we are providing covered services in accordance with their policies and expectations.
  • With other health care professionals you are in treatment with, so that your services can be coordinated with your health care team to provide better care
  • For minors age 13 and above, providing information about payment for services and appointment scheduling and transportation.  See your therapist's disclosure statement for specific information about treatment for minors and the age of consent for treatment in Washington state.
  • In consultation or supervision of your services with other professionals in our organization who are also bound by confidentiality laws and ethical codes.

We value teamwork and welcome coordination of care and collaboration requests from other health care professionals.  Please fax Release of Information forms to 425-367-0550 and let us know how we can work together to support our client's health. 

For Clients:  ​  ​
​You always have the right to review your personal health record and you will regularly collaborate with your therapist about your goals for treatment and your progress.  Please feel free to ask your therapist to review your file with you and we will provide a copy of any document at your request.  We will also communicate with you about any requests for information about you from a third party (like your PCP)- even if you signed a release of information form.
